Advance Care Planning: what matters most for your future care?
An Advance Care Plan tells your loved ones and healthcare team what healthcare you want, or don’t want.
Having a plan makes it much easier for everyone – especially if you are no longer able to make healthcare decisions for yourself.
The first step is to think about what is important to you?
Then talk about it with your family and healthcare team - put your wishes in writing and share a copy with your whānau and GP practice.
Head to the My ACP website and find out how you can put your own Advance Care Plan together at www.myacp.org.nz...
