An Advance Care Plan tells your loved ones and healthcare team what healthcare you want, or don’t want.
Having a plan makes it much easier for everyone – especially if you are no longer able to make healthcare decisions for yourself.
The first step is to think about what is important to you?
Then talk about it with your family and healthcare team - put your wishes in writing and share a copy with your whānau and GP practice.

Calling all those in Wheelchairs
Hi, I'm Simon .... a 44 yr old wheelchair user who wants to meet others in a similar position as myself (particularly a woman). I've been a sole parent now for almost 18 years and my son has just left the nest, so I thought it was time to start on the next stage of my life. I have cerebral palsy that effects just my legs (kinda the reason I can't walk) sooooooo if any of you are interested in getting to know me, flick me a message. I enjoy spending time with my son when he comes to visit and my pets (1 cat and 2 birds), I also enjoy building computers as well as a bit of gaming/watching TV too.
