Three key failures led to dead animals in Akaroa's water supply
The Christchurch City Council has apologised to residents and business owners after an investigation found a series of mistakes were to blame for them having to boil their water for 35 days from February 12.
An internal investigation into the incident, released on Monday, was critical of staff from the council and Citycare, which is responsible for the operation of the treatment plant and reservoir.
One council staff member had received formal disciplinary action, but council three waters head Helen Beaumont could not say what action was taken. The council had also asked Citycare to explain its actions, and it wanted the company to conduct a similar review of its staff. Read the full story here.
