Passed away peacefully at Telford Rest Home on Tuesday, August 14, 2018, aged 74 years. Dearly loved mother of Rocky and Joanne Soffe. Loved Nana of Kayla and Serena. Loved partner to Brian (dec). Pat's family wish to express their gratitude and thanks to the staff at Telford Rest Home for their love and care shown to Pat in her final days. According to Pat's wishes, a private cremation has been held.
